Physical Therapy Prescription

If you are seeking the benefits of physical therapy due to an accident, injury, illness, pain, pre or post surgical rehab conditioning or other diagnosis, you may print out a copy of our prescription for your physician's approval. Most insurance companies require a physician's diagnosis and signature on a prescription to initate a physical therapy or aquatic therapy program.
